We are honored that you have chosen Her Wellness Collective for your care.
Our practice was created to provide thoughtful, personalized care for women navigating the hormonal transitions of life. Because our visits are intentionally extended and highly individualized, preparation prior to your appointment allows us to dedicate our full time to meaningful discussion, thoughtful evaluation, and a clear treatment plan.
To ensure the best possible experience, we ask that all new clients complete the steps below at least one week prior to their scheduled appointment. Completing these items in advance allows us to review your history carefully and ensures that your visit can be devoted entirely to your care.
Before your visit:
- Complete your New Patient Intake Form – this was sent to your email when you booked your appointment. Please reach out to us if you did not receive it and/or need us to resend it. Please set aside 15 – 20 minutes to thoughtfully complete this form as it cannot be saved and all work will be lost if the page is left prior to submitting.
- Review and sign practice consents and policies. These forms were sent along with the intake form upon booking. Please reach out if you have any questions/concerns while completing these forms or if you need any additional copies.
When it’s time for your visit:
- At the time of your appointment, click the appropriate button below to enter your virtual waiting room with Her Wellness Collective. This link can also be found in your appointment reminders. Your provider will start your video visit shortly after you have entered the waiting room. Please check that your microphone and speakers are working and if you are having any technical issues, text or call us at (262) 239-7327 and we will assist you in any way we can.

After your visit:
- If you have not paid your invoice, please do so now. An invoice was sent to your email with instructions for payment or you can use the following link: https://app.elationemr.com/pay/herwellco.
- Download the Spruce App to communicate with your provider on a HIPAA-compliant platform. This app is free! Your provider will call you for a follow-up in 4 weeks; if this call hasn’t been scheduled yet please reach out to us. You will also have unlimited access to your provider for the 4 weeks between your initial consultation and your follow-up phone call. Feel free to reach out to us with all of your questions and concerns!
- A superbill or itemized receipt can be provided upon request. This can be used to submit to insurance or for FSA/HSA reimbursement.
